August 2, 2026
Dear Parents and Guardians:
As we begin the 2026-2027 school year, Cambridge City Schools is committed to providing a safe and healthy learning environment for all students. This letter serves as an important reminder regarding student immunization requirements and completion of Final Forms.
Students who are not in compliance with Ohio immunization law and/or do not have all board policy required Final Forms documents completed by both a parent/guardian and student will be excluded from school effective September 4, 2026.
Excluded students will not be permitted to:
Attend school;
Participate in extracurricular activities;
Attend athletic practices or contests;
Participate in field trips; or Participate in any school-related function until compliance is achieved.
Ohio law requires students to maintain required immunizations or have appropriate documentation on file with the District. Compliance with immunization requirements is essential to protecting the health and safety of all students and staff.
In addition, the District requires all parents and guardians to complete required forms in the Final Forms portal annually. The Final Forms portal contains critical safety, medical, emergency contact, transportation, and authorization information that school personnel rely upon to ensure student safety. Completion of all required documents in the Final Forms portal is required by Board of Education policy.
If you have questions regarding immunization requirements, please contact your child's school nurse or school office. For assistance with Final Forms, please contact your child's school office.
We appreciate your prompt attention to this matter and your partnership in helping us maintain a safe and successful school environment for all students.
Sincerely,
Dan Coffman
